GUIDANCE FOR HEALTHCARE PROFESSIONALS

IT’S MORE THAN JUST PRESCIBING – COLLABORATIVE CARE

“Coming together is a beginning, staying together is a process, and working together is success.” – Henry Ford
Prescribed Alternatives  harm reduction is more than just prescribing medical alternatives to the toxic drug supply. Many patients who engage in the program require stabilization through focus on the social determinants of health. With patients in the program, we see increased access to supportive services such as primary care, housing, social support programming and infectious disease management. Access to these wraparound services, either on site or via referral is an important consideration when offering Prescribed Alternatives.
