Primary Care
Some patients who participate in Prescribed Alternative harm reduction have an established primary care provider. When patients are already engaged with a primary care provider, continuous information sharing is encouraged to promote collaborative and safe care. This is particularly important when:
- Patients have co-morbid physical health conditions
- Patients have concurrent disorders
- Patients are prescribed psychoactive substances
- Patients have upcoming health events which may impact their substance use, or require titration (i.e., surgeries, injury, mental health concerns, adverse social events).
Regular check-ins and note sharing are recommended between the Safer Supply Team and the primary care provider.
