Pharmacy

Open communication with the patient’s preferred pharmacy is important for collaborative care. When patients are given Prescribed Alternatives, the pharmaceutical is picked-up daily from the pharmacy. This means that the pharmacy is likely in contact with the patient the most out of all providers. Pharmacies not only dispense the patient’s medications and fax missed doses, but also provide valuable input to the Prescribed Alternatives team on the patient’s status and changes in their health.

Recommendations for pharmacy:

  • Promote an approachable and supportive environment for individuals on Prescribed Alternatives.
  • Recognize and plan for the potential increased for patient with concurrent disorders.
  • Engage in regular communication with the team about any clinical concerns for the patient.
  • Fax any missed pick-up days as soon as possible to the team involved with the patient.
